How much Bitcoin is effectively "dead" forever?

Out of the ~19.8M BTC mined so far, estimates say millions are lost in forgotten hard drives and locked wallets.

​If we subtract lost coins and diamond hands, what's the true available supply being traded on exchanges today? Are we trading the same few million coins back and forth?
